Jeremy West, 35, of Amery died as the result of injuries sustained in a two-vehicle accident Thursday, April 26.
Polk County Sheriff's deputies responded to a report of a crash in the 1700 block of 120th Avenue in the Town of Garfield at around 7:15 p.m. Thursday.
A 1995 Pontiac Grand Prix and a 1999 Mercury Cougar were involved in the accident. According to the sheriff's report, the Grand Prix, driven by Douglas Wilberg, 50, Amery, was eastbound on 120th Avenue. The Mercury Cougar, driven by West, was westbound on 120th Avenue.
The West vehicle was reportedly traveling at a high rate of speed at the time of the accident.
According to the accident report, when West's vehicle crested a hill, the driver lost control of his car and collided with Wilberg's vehicle.
The drivers were the lone occupants of each vehicle. Wilberg was wearing a seatbelt and sustained minor injuries. West was allegedly not wearing his seatbelt and he was pronounced dead at the scene by the Polk County Medical Examiners Office.
The crash remains under investigation. The sheriff's office was assisted at the scene by the Balsam lake and Dresser/Osceola/Garfield Fire Departments, and the Amery and Unity ambulance services.
The crash is the fourth fatal accident in Polk County in 2012. All four have been in April.
In 2011 there were a total of seven fatal accidents for the entire year, according to Sheriff Peter Johnson.
